iFRIENDS AGE-RELEASE FORM & DOCUMENTATION PACKAGE
iFriends engages only those webcam Video Chathosts – or Chathost Representatives – who prove themselves to be at least 18 years of age and who provide iFriends with a complete identity documentation package, including without limitation official photo ID. Each such individual (Video Chathost and Representative alike) must complete and submit this form, and respond to each question. (Blank answers are not accepted; if a question does not apply to you, answer "N/A" or "Not Applicable").
Each individual who wishes to appear in any video chatroom on iFriends, and/or who wishes to register as a Chathost Representative, is required to complete this form in its entirety, print it, sign it, and mail it to iFriends (see “HOW TO SEND” below), along with at least two forms of ID (see “PHOTO ID” below). If multiple people wish to appear in your video chatroom, then multiple forms must be printed, signed and mailed to iFriends - each person, and each Chathost Representative, must fill out their own, individual form and each person appearing in a video chatroom must go through our Age Verification procedures. Every person, before they can appear in a video chatroom, must be individually and specifically approved by iFriends directly.
Photo ID
Each individual who completes this form must also provide to iFriends at least two forms of identification. The first is your “Primary Photo ID”, which must be an unaltered driver’s license, passport, or some other form of official government-issued photo identification, bearing a recent photo, your signature, and your date of birth. If you are located outside the United States or do not have a U.S. Social Security Number, and you are under the age of 25, your Primary Photo ID must have been issued within the last three years. Regardless of where you live, if you are 21 or younger, your Primary Photo ID must be issued at a time when you were at least 18 years of age. You must also provide at least one secondary identification document, such as college ID, birth certificate, SSN card, or government healthcare card. Note: If you are a college student, we strongly suggest that your secondary ID include your current college ID; if you host in the mature-themed areas of the service, this will result in your exposure in the highly popular “College Girls” and “College Hunks” areas of the service.
Quality of Photo ID: In general, the more photo IDs you send iFriends, and the greater their quality, the greater the odds are that iFriends will promptly approve your materials.

(Note: If you intend to use the Early-Approval Uploader capability to send iFriends your photo IDs, owing to legibility issues, iFriends does not at this time accept photo IDs via fax. iFriends accepts photo ID only via the real-time, internet-based Uploader form (early approval), and via postal mail (final approval).

How To Send
Early-Approval Option (Preliminary Approval): If you have your photo IDs and a signed version of this form and the Video Chathost Agreement (and Chathost Representative Agreement - if you are working with a Chathost Representative) on your computer, you can upload these materials to iFriends using your web browser. Doing so may result in same-day preliminary approval, and may enable you to begin hosting videochat room sessions on iFriends immediately. For more information on this option, select the "Documentation" link from within your ID-Card Manager and choose the “Early-Approval Uploader” option.
Final Approval: All chathosts and/or Chathost Representatives -- including chathosts who use the Early-Approval Uploader -- must mail or overnight to iFriends the original printed version of this signed form, the Video Chathost Agreement (and Chathost Representative Agreement - if you are working with a Chathost Representative), plus legible copies of all photo IDs.

Notice: In the event that you fail to promptly mail to iFriends your complete, executed documentation package (including signature), iFriends reserves the right to suspend hosting privileges, or terminate your account. To avoid interruption or inconvenience, please mail or overnight your full documentation package to iFriends as quickly as possible.
Age Certification Consent To Release Age-Certification Information
I hereby certify that attached herewith are at least two forms of identification which state my true date of birth, indicating that I am at least 18 years of age, or the age of majority in my legal jurisdiction (whichever is greater). My signature also certifies that, should I provide WebPower/iFriends with false identification (including name, image, and/or date of birth), I will not hold WebPower/iFriends liable for the direct or indirect results of my fraudulent actions, and I will indemnify WebPower/iFriends to the greatest extent permitted in law or equity, for any actions or damages that result from my fraudulent behavior, as I understand that this identification was accepted in good faith, and that this effort was made to ensure that I am at least 18 years of age, or the age of majority in my legal jurisdiction (whichever is greater).

I also certify that all decisions related to the content and nature of my videochat sessions/Productions are made at my own discretion, and that these videochat sessions/Productions do not violate the local community standards for “obscenity” in my area, and that my videochat sessions/Productions do not violate any other laws, domestic or foreign. I am completely responsible for my own actions. I hereby indemnify and hold harmless WebPower/iFriends from all related liabilities. I hereby discharge all persons related to WebPower/iFriends of any past, present or future liability.

I further represent that I have fully read this form prior to its execution and that I have not been induced or coerced to sign this form by any statement made by iFriends, its agents, employees, consultants, or anyone acting on their behalf.  I hereby knowingly, willfully and voluntarily consent to allow full and complete access by WebPower/iFriends, or its agents, representatives or designees (collectively, "WebPower/iFriends"), to all available records (the “Records”) that support my claim that I am at least 18 years of age or the age of majority in my legal jurisdiction (whichever age is greater). The Records include, but are not limited to, birth certificates, drivers licenses, passports, school records and such other records that verify or tend to verify my true birth date, age, or identity, as may be available from any person or governmental, religious or private agency, organization or other entity.

Further, I hereby expressly authorize any person or governmental, religious or private agency, organization, or other entity, to provide the Records to WebPower/iFriends and I hereby release any such person or entity from any liability for acting in reliance on this Consent.

I understand that any information obtained by WebPower/iFriends from the Records is used for WebPower/iFriends' internal purposes and that such information will not be disclosed to any party, other than WebPower/iFriends and its authorized representatives and agents acting on the company’s behalf, unless WebPower/iFriends is compelled by law to do so by a law-enforcement agency, other authorized governmental agency or court of law.
