DIRECTIONS: Print out this form, complete it, sign it, and fax it to 1-415-354-3327 (International Customers dial 001-415-354-3327). Shortly following receipt of your fax, we will email the primary email address you provide below with confirmation of receipt. If you do not receive a confirming email within a few hours after transmitting it, please re-fax the form.
If accepted, a iFriends customer service representative will promptly perform the necessary indications on your account profile, upgrade and re-activate your account, and notify you via email of same.
Note: If you do not have access to a fax machine, you may mail the signed agreement to Attn: Preferred Customer Agreement Dept., 3395 S. Jones Blvd., Suite 22, Las Vegas, NV 89146 USA. However, we will not be able to reactivate your account until we receive your mail.
Although our customer service department cannot accept or approve your agreement over the telephone, you may call for general questions or inquiries 24 hours per day, seven days per week, toll-free at 1-800-437-4363 (callers outside the U.S. and Canada, click here for toll-free dialing instructions from your country.) or via our online contact form.

INTERNATIONAL CUSTOMERS: As part of our verification process, we will contact your financial institution to verify your address information. This process may hinder the reactivation of your account; therefore, we offer our customers another option. You can completely bypass this step by faxing us a copy of a recent credit card statement. The credit card statement must show your name, address and credit card number. You may black out any other information that you would prefer we not see.
Billing: As a frequent and active user of this Service, I accept responsibility for all activity and charges related to my account. If this Service is unable to collect payments for charges related to my account, I understand that each payment method that I have provided to this Service may be re-billed until such amounts are collected. I authorize this Service to disclose my personally-identifiable information to third parties for the purpose of proving the legitimacy of any charge. I understand that my credit rating may be adversely impacted if I fail to make timely and sufficient payments to this Service or to any third party collection agency.
Fraudulent Activity: I agree that I will only provide this Service with payment methods which I am authorized to use. If I use this Service or any payment method fraudulently or without authorization, I acknowledge that this Service may disclose my personally-identifiable information to law enforcement agencies. Even if my use of this Service is fraudulent or unauthorized, I remain personally responsible for all charges related to my account and acknowledge that I may be subject to litigation.
Cancellation: I am aware that I can cancel my account at any time. To cancel my account, I will contact this Service directly by using the Online Cancellation Form or the Online Help Assistant, by calling Customer Care at 1-800-437-4363, or by sending a written cancellation notice to Customer Care at 3395 S. Jones Blvd., Suite 22, Las Vegas, NV, 89146, USA.
